RN Case Manager (Hospice) needed for Oahu. Mon – Fri (8:00am – 4:30pm).
- Must have at least 1 year experience as an RN (Hospice preferred)
- Reliable vehicle and able to commute throughout Oahu (mileage reimbursement provided)
- Flexible schedule required and open for after hours on-calls and available weekends

Responsibilities of the RN Case Manager (Hospice)
We are currently seeking a Nurse Case Manager to utilize principles of palliative care and holistic philosophy to provide care that reflect skilled nursing judgement, medical necessity, measurable patient caregiver objectives and reasonable goals. The primary duties include but not limited to provide case management services to best benefit patients and caregivers; care provided reflects compliance with federal and state regulations; documents all nursing and case management activities in accordance with hospice protocol; participates in Interdisciplinary Team conferences and may be required to assume on-call duties after hours on holidays or weekends as needed.
Requirements of the RN Case Manager (Hospice)
- 1 year of recent experience as an RN (Hospice preferred)
- Hawaii License
- Must have BLS card
- TB Clearance
- Immunization Records
