Job Title: Regional Infection Control Nurse and Safety Coordinator

This organization is a chain of healthcare facilities providing services on several islands.

Description:

· Administrative and Surveillance Functions – Infection Control

o Plans, organizes, implements, and maintains a hospital-wide infection control program and activities

o In collaboration with key Hospital departments and medical staff, develops and implements a system for identifying, investigating, reporting, and preventing the spread of nosocomial infections among the residents and healthcare personnel of Leahi Hospital and Maluhia using established criteria and definitions as a guideline

o Assists in the development, implementation and improvement of hospital-wide infection control policies, procedures, and practices

o Designs and implements new methods to improve the efficiency and outcomes of the Infection Control Program

o Makes recommendations to Department Managers in the development of their department specific infection control policies, procedures, and practices Annually reviews and/or revises departmental policies and procedures related to infection control

o Coordinates infection control surveillance activities hospital wide

o Identifies and collects data on patients with hospital-acquired infections; identifies patients at risk of acquiring an infection; identifies and analyzes risk factors associated with the development of hospital-acquired infections

o Identifies outbreaks of infection and confers with medical and nursing staff to determine appropriate implementation of isolation precautions including culture and sensitivity and/or isolation precautions when indicated

o Reviews cultures and sensitivities reported by microbiology laboratory of patients and personnel to determine if infection is nosocomial in origin

o Prepares and reviews nosocomial infection data for unusual epidemics, clusters of infections, infections due to unusual pathogens and any occurrence of nosocomial infections that exceed the established threshold

o Maintains accurate records of nosocomial infections among patients and personnel

o Initiates contact investigation to determine potential exposures within the facility and initiates recommendations to control and prevent the transmission of infectious and communicable diseases

o Reports required communicable diseases to the Department of Health

o Conducts chart reviews and confers with staff nurses on all resident units on a regular basis to determine occurrence of nosocomial infections

o Conducts or participates in special studies concerned with the prevention and control of hospital-acquired infections

o Monitors methods of asepsis, sterilization and disinfecting employed throughout the facility

o Participates in the review and selection of germicides, cleaning products, personal protective equipment, disinfectants, sterilant, and safety devices in use throughout the facility

o Reviews and revises Infection Control policies and procedures to maintain standards of practice

o Participates in hospital-wide performance improvement activities as assigned

· Administration and Surveillance Functions – Safety

o Plans, organizes, implements, and maintains a hospital-wide safety program and activities

o Develops a system for identifying and ensuring the safety of residents, staff and tenants at Leahi Hospital and Maluhia

o Assists in the development, implementation and improvement of hospital-wide safety policies, procedures, and practices

o Designs and implements new methods to improve the efficiency and outcomes of the Safety Program

o Makes recommendations to Department Managers in the development of their department specific safety policies, procedures, and practices

o Implements and evaluates surveillance activities using established definitions and criteria for the identification and compliance in safety standards of OSHA, CMS, National Patient Safety Goals, NFPA, etc.

o Coordinates safety surveillance activities hospital wide

o Collects data on injuries, safety hazards and presents comparative data analysis at appropriate facilities

o Identifies safety hazards and assesses appropriate corrective action plan

o Participates in hospital-wide performance improvement activities as assigned

o Conducts Environment of Care (EOC) rounds on a regular basis to monitor compliance with safety standards

o Develops, reviews, and revises the Infection Control and Safety Program manuals on an ongoing basis

o Communicates to department managers and staff safety related issues and updates

o Coordinates and facilitates the Safety and General Health Committee

· Employee Health

o Works with the Employee Health Nurse to implement the hospital’s infection control policy if an employee is exposed to TB or other infectious disease

o Meets with the staff to assist in identifying and correcting problem areas, and/or the improvement of services, as necessary

o Develops and maintains a good working rapport with all department directors and staff to ensure that infection control services and activities can be properly maintained to meet the needs of the residents

o Develops, maintains, and keeps current safety program Actively participates in safety and infection control in-services and new employee orientation

· Committee Functions

o Serves on, participates in, and attends Infection Control and Safety and Emergency Management Committees meetings for Leahi Hospital and Maluhia

o Works with the Infection Control and Safety Committee to evaluate and revise the facility and individual department’s practices and policies and procedures

o Consults with Infection Control Committee Chairperson when investigating infection clusters and unusual pathogens

o Submits targeted surveillance indicators, based on studies that identify areas for improved patient outcomes, for committee’s approval

o Provides the Infection Control Committee and Administration with written and oral reports concerning all infection control problems within the facility

o Serves as a liaison between the facility Infection Control Committee and other health professionals regarding infection control activities

o Prepares, analyzes, and reports infection data to the Hospital Quality Review Committee, Nursing Department, Safety Committee and Medical Staff

· Other Duties

o Attends and participates in mandatory in-service educational classes and on-the-job training programs as directed

o Participates as a contributory team member of Oahu Region

o Attends departmental staff meeting, Nurse managers’ meetings and other meetings as requested and/or assigned

o Performs other related duties as assigned

Requirements:

· College Degree from an accredited school of nursing, BSN and master’s degree in Nursing or health related field is preferred

· Must have at least one and a half (1-1/2) years of professional nursing work experience in performing a variety of nursing care preferably in the long-term care arena requiring the knowledge of and ability to comply with Medicare/Medicaid rules and regulations

· Must have at least one-half (1/2) year of supervisory experience which included:

o Planning and directing the work of others

o Assigning and reviewing their work

o Advising them on difficult problem areas

o Timing and scheduling their work

o Training and developing employees

· Supervisory experience in covering several nursing units independently is preferred

· Possession of a master’s degree from an accredited college or university school of nursing may be substituted for one year of the required experience

· Possession of a valid, State of Hawaii Registered Nurse registration/certificate

· APIC certification is desired

· Extensive experience in infection control – writing policies and procedures; conducting infection control surveillance studies; collecting data resulting in data analysis; participating in educational activities and interdisciplinary team meetings; developing action plans and establishing and maintaining effective working relationships with residents, staff, physicians and others

· Ability to independently perform nursing care for residents

· Experience in safety activities; familiar with infection control and safety regulatory standards; writing policies and procedures; conducting safety inspections; participating in educational activities and maintaining effective working relationships with residents, staff, physicians, and others
