Pay: $80,784k/yr  

Experience: 2 years minimum clinical or related experience; Experience in the rehabilitation of chronically/catastrophically ill or injured individuals 

Education: 

  • Graduate from an accredited school of Nursing, Physical Therapy or Occupational Therapy or Master’s Degree in Speech Language Pathology Certified Case Manager
  • Current license as a Registered Nurse, Physical Therapist, Occupational Therapist or Speech Language Pathologist in the State of Hawaii
  • Current BLS certification

Type: Full-time; Direct Hire 

Schedule: Monday – Friday, 8 a.m. to 5 p.m.

Kahu Malama Nurses is seeking a Community Liaison to join a growing and dynamic team!  

 

Job Description: 

  • Perform activities and duties related to potential patient identification, pre admission assessment screening and admission of patients to services along the rehabilitation continuum of care
  • Identify individuals who meet program criteria and will benefit from program services
  • Promote and market REHAB’s programs and services
  • Serve as a clinical practitioner, educator, advocate, consultant and collaborator

Position Requirements: 

  • Excellent assessment skills and understanding of insurance and managed care
  • Demonstrate expertise in analysis and decision making
  • Effective written and verbal communication skills and time management skills
  • Familiarity with the resources available for use in assessing regulations and the parameters of third-party reimbursement
  • Valid State of Hawaii driver’s license
